1. Get your Cedula
2. Get NBI Clearance or Police Clearance
3. Order of Payment (Mayor's Permit)
Go to the Basement of Civic Building B (located at the back of the Quezon City Hall Main Building -ask the security officers) and show your Cedula and NBI Clearance or Police Clearance. The evaluator will give you the Order of Payment for the Mayor's Permit/ Occupational Permit.
4. Order of Payment (Health Card/ Medical Certificate)
It is just beside the table of the Occupational Permit Order of Payment. The evaluator will give you the Order of Payment for the Health Fee.
5. Payment
Go to City Treasurer's Office Miscellaneous Fees Division (you have to exit the building, it is near Land Bank) and pay the following:
Mayor's Permit: PhP 150
Picture: PhP 20
You can also pay the required fee for the Health Card.
6. Occupational Permit Application Form
Go back to Basement of Civic Building B and show your Cedula, NBI Clearance or Police Certificate and your Occupational Permit and Medical Certificate receipts. The evaluator will attach your documents with the Occupational Permit Application Form. You need to fill-in the Occupational Permit Application form and submit everything to Counter 9 or 10 located in the Ground Floor of the same building.
7. Picture Taking
After submitting your documents in Counter 9 or 10, your name will be called for the picture taking. Note that sleeveless top is not allowed.8. Releasing
Congratulations, your name will be called for your Quezon City Occupational Permit.

ReplyDeletei don't think cedula should be from qc. they did not get the original nbi copy, they just needed to note the receipt number of your nbi clearance.

2. Can you use a medical certificate from your employer/company? For example so long as it's intellicare accredited?
3. Can you choose your schedule for the HIV seminar? Do they have on schedules on Saturdays?

Hi Sarah! Here are the answers to you questions.
Delete1. Normally government offices are closed on Saturdays.
2. I think that's not an option at all. You have to get the medical certificate from Quezon City Health office.
3. Normally you can choose the schedule for the HIV seminar on the next day, not on the same day.
They don't have a schedule on Saturday. An option is to write a letter scheduling a seminar for a group of people, a company maybe (not an individual request). I just don't know what it entails like if you're going to provide the venue and maybe tokens for the speakers...
Please note, I am speaking based from observations and actual experience but I'm not a Quezon City Hall employee. Please visit their website or call them to verify because I can be wrong. Thanks.

Well, the requirement for Occupational Permit includes getting your Medical Certificate/Health Card. You can simultaneously do it I think, since as soon as they see that you paid for getting a Medical Certificate/Health card they will process your Occupational Permit. Diskarte lang my friend. You can figure it out.

ReplyDeleteAlmost the whole day. Depende dun sa time na pagdating mo sa Quezon City Hall. The earlier, the better. Of course, depende rin sa dami ng tao and season. When I got mine, it was October na so konti na lang kumukuha.

Laboratory (if instead you want to go to accredited laboratory then it depends): 94

My question is, do I have to get a Police Clearance from Quezon City or will a Police Clearance from Marikina suffice?
